Nominations are now being accepted for entry in to the St. Croix Central High School Hall of Fame. The Athletic Hall of Fame will recognize individuals in three nomination categories: Athletes, Coaches and Distinguised Service. Nominations may be submitted by members of the community, general public and Athletic Hall of Fame Committee members. Deadline for nominations is March 1st. Selection of recipients will be completed on or before June 1 and a formal awards ceremony will be held in the SCC auditorium the Saturday after Homecoming with a banquet to follow. All Boys' HS Blackhawk Hockey players & Fusion HS Hockey players' (members of BHA) will pay their $450.00 Blackhawk Hockey Association (BHA) fee to the Baldwin-Woodville School District. Please make the checks out to: Baldwin-Woodville Schools.

2016-2017 Physical Dates
If you had a physical on or before 3/31/15 - you will need a physical and must complete the green physical form
If you had a physical between 4/1/15 and 3/31/16 - please complete the yellow physical form
If you had a physical on 4/1/16 or after - you do not need a physical form
All 6th graders need to have a physical and complete the green physical form
If you are unsure if your child needs a physical, please feel free to call the high school or middle school office.

Requirements for Participation in Athletics at SCC
To participate in athletics offered by the St. Croix Central School District, a student and their parent(s) or guardian(s) must provide the following information prior to the first day of practice. These are required as members of the Wisconsin Interscholastic Athletic Association.
In returning the SCC Parent-Student agreement form, an individual is stating that they have read and understand both WIAA and SCC regulations surrounding the privilege of participation in athletics.
By WIAA policy, any athlete and/or parent/guardian not returning a signed agreement will not be allowed to participate in high school athletics.
The following forms must be on file in the Main Office at the High School or Middle School before a student will be allowed participation in practice or competition for an athletic program at SCC. Links to forms are at right. >>
SCC Parent-Student Agreement Form must be completed every year, all 6-12 grade athletes.
A Green Physical Examination Card or Yellow Alternate Year Card
Completion of the ImPact Concussion Test - all 6-12 grade athletes
Athletic Fee Collection Form
Onsite at SCC Middle or High School, or at a medical facility, conducted by licensed and trained medical staff
Once every two (2) years unless directed otherwise by medical staff

All student athletes and parents are required to read the SCC Athletic Code Handbook. When student athletes and parents sign the Yellow Alternate Year Card or Green Physical Card, they are acknowledging that they have read and understand what is contained in this document.
