Requirements for Participation in Athletics at SCC
To participate in athletics offered by the St. Croix Central School District, a student and their parent(s) or guardian(s) must provide the following information prior to the first day of practice. These are required as members of the Wisconsin Interscholastic Athletic Association.
In returning the SCC Parent-Student agreement form, an individual is stating that they have read and understand both WIAA and SCC regulations surrounding the privilege of participation in athletics.
By WIAA policy, any athlete and/or parent/guardian not returning a signed agreement will not be allowed to participate in high school athletics.
The following forms must be on file in the Main Office at the High School or Middle School before a student will be allowed participation in practice or competition for an athletic program at SCC. Links to forms are at right. >>

SCC Parent-Student Agreement Form must be completed every year, all 6-12 grade athletes.

A Green Physical Examination Card or Yellow Alternate Year Card

Completion of the ImPact Concussion Test - all 6-12 grade athletes

Onsite at SCC Middle or High School, or at a medical facility, conducted by licensed and trained medical staff

Once every two (2) years unless directed otherwise by medical staff
